We will not retaliate against you for filing a complaint.

Website versus clinical records

Messages you send from this website by email are not a secure patient portal. Do not email social security numbers, full insurance IDs, or detailed medical histories. The Privacy Policy covers website data separately.

Changes to this notice

We may change this notice and make the new terms effective for all PHI we maintain. The current notice will be posted at the clinic and on this page.
